Documents to Bring to Your Appointment

Three things matter most: valid government photo identification (an unexpired passport is best, a driver's license works), any immunization records you hold — foreign-language records are perfectly fine — and documentation of any significant medical history, particularly a previous positive TB test or treatment for a chronic condition. Add a written list of medications you currently take. If you have already printed Form I-693, leave it unsigned; the signature must be made in front of the civil surgeon. Why Accuracy Is What You're Actually Paying For

Newark generates more immigration filings than any other city in New Jersey. An I-693 is a legal document that a physician happens to complete, and its only job is to be accepted by USCIS the first time.
